Cobble Hill Health Center is a not-for-profit long- and short-term care facility providing services to the disabled and elderly for over 30 years. The Cardiopulmonary Rehab Manager will play an integral role in helping us to achieve our vision: to consistently exceed expectations and set the standard for excellence in residential and community care.
The Cardiopulmonary Rehab Manager must be an exceptional leader with a track record of creating high performing teams and excellent team culture. They will need to be collaborative, team-oriented, self-motivated and driven, with a strong sense of ownership for the cardiopulmonary rehab program.
Cardiopulmonary Rehab plays an integral role in caring for and assisting patients who are recovering from and managing their cardiovascular and pulmonary issues including but not limited to myocardial infarction, open heart surgery, percutaneous coronary intervention or recent hospitalization for heart or respiratory failure. The cardiopulmonary rehab model applies to patient populations with COPD, CHF, CAD with or without comorbidities such as diabetes, renal insufficiency, sleep apnea, anemia and others. Its aim is to provide continued best in class guideline directed medical care outside the hospital setting while reducing rehospitalizations.
The ideal Cardiopulmonary Rehab Manager will:
Assist in creating the cardiopulmonary rehab program
Assist in developing nursing protocols for CHF and other cardiac conditions based on evidence-based practice to ensure consistency and quality of care
Assist in establishing a small, 6 bed progressive care unit within the cardiopulmonary unit that will serve as a step-up unit for patients that need closer clinical monitoring with the ability to push medications to reduce re-hospitalizations
Create and maintain relationships with partnering hospitals to enhance our program and share in the care of our patients
Involvement with inpatient and outpatient settings to assist in setting our patients up to succeed through their continuum of care
Develop strong discharge protocols and education for the patient's success beyond their rehab stay
Be a strong educator to guide his/her staff to be proficient in the areas of nursing care as it relates specifically to cardiopulmonary assessment, nursing diagnosis, planning and implementation
Incorporate into practice the generally accepted new techniques and knowledge acquired through continuing education and evidence-based practice
Utilize equipment and technology skillfully while being technically competent in nursing procedure
Maintain a strong and calm composure in the face of stress and emergency situations
Serve as a resource to nursing staff, nursing students and other health care personnel, and the community
The ideal candidate should have:
Have a baccalaureate degree in Nursing from an accredited nursing program
Be licensed in the state of New York
Be certified in BLS and ACLS
5 years of experience in CCU/ICU, coronary step down units or Cardiac Rehab program with a minimum of 2 of those years in a leadership role
Experience in EKG interpretation, medic procedures and cardiac emergencies
