Nurse - Full time Day Shift

Nurse - Full time Day Shift will be available to remotely monitor participants and work with members of the clinical care team to triage clinical concerns identified and escalated by the wellness coach. Telehealth nurse will remotely evaluate participants, identify early signs of deterioration, engage with ancillary care team, and escalate concerns to provider for further evaluation. The Nurse will think critically and creatively about what is needed to enhance health improvement and will use excellent interpersonal skills to work successfully with participants and the members of the clinical care team. The nurse will take the initiative to ensure clinical care and related support services are made available to participants.

Responsibilities

Triage participant concerns when alerted by Wellness coach by assessing severity of symptoms.
Review alerts when notified by Wellness coach of a change in condition and communicate with appropriate members of the clinical care team.
Document actions and interventions.
Educate participant on their disease process.
Recognize urgent or emergent participant care situations, seek assistance of the medical provider, and initiate appropriate emergency interventions as directed.

Requirements
Bachelor's Degree in Nursing, preferred.
Cardiology and Care Management experience preferred
Current license to practice as a Registered Professional Nurse.
Proficiency with Windows-based computer applications, email and web-based skills.
Ability to learn and work in multiple EMRs
Must be able to work from home with a HIPPA compliant environment.
Must be available to work some holidays and weekends.
